Your gender identity and place of birth
We collect this information to ensure that we are inclusive and equitable in our review and selection process and that we uphold our commitment to recruit a diverse group of Fellows.

A "gender pronoun" is the pronoun(s) that a person chooses to use for themselves. For example: If Xena's gender pronouns are 'they', you could say "Xena ate their food because they were hungry."

The Opportunity Network will not release information about the identifiable immigration status of students unless consent is given or we are compelled to do so by law. We will do everything in our legal authority to protect the privacy of our community.

Your racial/ethnic identity

In this section, we will be asking you to provide us with a bit more information about how you identify.  At The Opportunity Network, we honor the identities and embrace the power of each member of our community. The following two sections allow you to describe, in greater detail, how you identify racially and ethnically.

Section 1

Select all the category(s) that best describe you. You may select more than one category, but you must select at least one.

Section 2

If you would like to, please use the space below to describe your racial and ethnic identity(s) in your own words (optional).

Your household information

Please list as much information as you can about both parents below, even if one or more is deceased or no longer has legal responsibilities toward you. We collect your responses to these questions to learn more about the support that you currently have for college and career and how OppNet might be a good fit for you. This information will inform the support that OppNet provides you with, should you be selected as a Fellow.

Why is OppNet asking about your household's income? OppNet uses this information as part of how we evaluate fit for our Program. If accepted, income information is used to provide individualized support to Fellows throughout the college guidance and summer opportunity placement processes. Note that, if accepted, OppNet will collect a copy of your parents'/guardians' most recent tax filings.
