By submitting my RSVP and/or participating in and/or attending this event, I grant to The Opportunity Network (OppNet), the right to take photographs and/or video of me in connection with the above-identified event. I authorize OppNet, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that OppNet may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising and Web content.

If I do not agree to grant OppNet permission to use photographs or video of me in the above mentioned manner, I will email volunteers@oppnet.org or alert an OppNet staff member when I enter the event and complete a Photo Opt Out Form to record my preference.
